Athletic Physicals on Horizon

All student athletes must complete a new packet of information for next year online before they will be allowed to participate next year.  This can be done online by clicking on the link below. Once completed it is on file with us.

Athletes will also need a new Pre-Particpation Physical Exam. They can print a new form by clicking on the link below. It must be dated AFTER MAY 1^st in order to be considered current for NEXT school year.

UIL Pre-Participation Physical Examination

We are holding physicals on Wednesday, May 15^th 5pm-7:30pm and  Saturday, May 18^th   9am- 12:30pm at All Star Orthopaedics. Tickets must be purchased in advance from CISD Athletics Website (letter attached).
